Please print and complete the forms and bring bring to your first visit. If you are unable to bring forms with you please come to the clinic at least 20 minutes before your scheduled appointment to complete the paperwork. ALL appointment that arrive 15 minutes or later than scheduled appointment will need to reschedule.
